The First Week Protocol
A survival guide for parents.
The day your child gets braces (Bonding Day) is exciting. They pick their colors, they take selfies, they feel grown up.
But 6 hours later, the biology kicks in.
As the wires warm up to body temperature, they begin to apply light, continuous pressure. For a mouth that has never felt force before, this triggers inflammation. The next 3 to 5 days can be challenging.
Here is the protocol we recommend to our families to turn a “tough week” into a manageable one.
1. The “Soft Food” Shopping List
Do not wait until they are hungry and hurting to figure out dinner. Stock the fridge before the appointment. Chewing will be tender, so we need nutrition without resistance.
- Protein: Scrambled eggs, protein shakes (with a spoon, no straws if possible to avoid suction pressure), Greek yogurt.
- Carbs: Mac and cheese, mashed potatoes, oatmeal, pancakes.
- Cold Comfort: Ice cream and smoothies act as natural anesthetics, numbing the gums while providing calories.
2. Pre-Emptive Pain Management
Do not chase the pain.
The soreness typically peaks 12-24 hours after bonding. We recommend taking an over-the-counter anti-inflammatory (Ibuprofen/Advil) immediately after the appointment, before the anesthesia wears off or the soreness sets in. Continuing this on a schedule for the first 24 hours prevents the inflammation spike.
Note: Always follow the dosage instructions on the bottle for your child’s age/weight.
3. The Wax Techique
We give every patient orthodontic wax, but few know how to use it correctly.
If a bracket is rubbing the cheek:
- Dry the bracket: Wax will not stick to a wet surface. Have your child suck in air or use a tissue to dry the metal.
- Roll a ball: Warm a pea-sized piece of wax between your fingers.
- Squish it: Press it firmly over and under the bracket wings.
4. The Psychology of “Permanent”
The hardest part for a child isn’t the pain; it’s the sensation of trapped food and the feeling that “this is forever.”
Reassure them: The mouth adapts incredibly fast. By Day 5, the cheeks will toughen up (keratinize), the pressure will fade to a background sensation, and they will forget they are even wearing them.
You just have to get them to Day 5.
